Memory leak-clinical focus on practical Neurology

These are excerpts from a review in the Medical Journal of Australia (MJA):

Elderly with mild cognitive impairment are at increased risk of progression to dementia but no test is useful in evaluating this risk. Medications are not favorable in mild cognitive impairment.

Physical activity and treatment of high blood pressure decrease the risk of dementia.

In people with Alzheimer's disease, a cholinesterase inhibitor or Memantine (a receptor antagonist, N-methyl-D-aspartate) provides a symptomatic relief. Drugs do not change the progression of the disease.

Psychological and behavioral symptoms are common in Alzheimer's disease.

Atypical antipsychotics reduce agitation and psychosis, but increase the risk of cardiovascular events.

Role in the management of depression antidepressant with mild cognitive impairment is uncertain, but they may increase the risk of falls and delirium.

6 Reasons doctors Blog

Here are 6 reasons why doctors Blog, according to Dr. John m., a cardiologist and hands:

Here are the top six reasons why I and other doctors choose to author blogs:

1. The practice of medicine guides
2. to educate
3. mankind better
4. in order to give you a look behind the curtain
5. to store useful information
6. to view our humanity

My comment is here:

Thanks for the wonderful post and sharing insights, John.

Blogging "better mankind" is beyond reach for me, I think. However, I hope that my blog has helped "educate" at least some of the readers who perused more than 8 million pages from 2005 ...

Doctors are highly skilled experts which limit their impact only to patients they see – if they don't publish, give lessons-and blogs. In most cases, the benefits far outweigh the risks and doctors should be encouraged to make at least one attempt.

I tried to describe a practical approach and efficient in terms of time:

Social media in medicine: how to be a superstar by Twitter and help patients and your practice
http://casesblog.blogspot.com/2011/10/social-media-in-medicine-How-to-be.html

Blogging also keeps grounded and humble. Critical comments require to run your clinical opinion, expressed in a post on the blog, with solid scientific references, and that is a good thing.

Sherri Vance, January 22, 2012: Dr. Ves, I love your blog. As regards the reasons why MDs should blog, I think the view behind the curtain (# 4) is the most important. If you are going to use MS Access 2003 don't forget to put your *.mdb file into App_Data dir.

I wouldn't go so far as calling .NET shitty, it's better than ActiveX any day of the week.

What I call shitty is the way Microsoft lured Anders Heljsberg away from Delphi so he could create C#. Anders was like a programming God to many of us, by him selling out, he lost his aura, he lost his glow. The .NET platform was more about slowing Sun's Java than to give me or you a really nice programming language, it's the way M$ works, protect the cash cow (Office) at any and all costs.

Utchin wrote:

Did you mean to say sidetrack wink

I know that the PunBB team could do anything they desire, after all, this is Open Source, scratch your own itch and all that. But creating a .NET equivalent of PunBB sends the wrong signal.

So, it's the fault of M$ and not the programmer who, (one can only assume), buggered off purely and simply for money, (or some other equally shallow motive)?

I doubt the overall Pun image can be tarnished much further than it already has been.

Here is an Anders Hejlsberg tribute page I put up back in 1998. The majority of the links are dead, but I saved the message that Anders sent to Compuserve on his departure and Chuck Jazdzewski's Usenet posting about Anders.

All you guys who are so discouraged because of PunBB.NET should understand that this project will make absolutely no harm to PHP PunBB. I've already wrote it here, but it seems to me i should do it again. PunBB and PunBB.NET are two separate projects. If you don't want to support PunBB.NET it's your choice. And we assure you that PunBB.NET won't replace PHP PunBB.

Since there are 2 developers working in punbb.net that are not working on punbb.org, there is the loss for punbb I think. Why have 2 teams while could have one bigger more powerfull team?

Also, punbb 1.3 took so long to launch, and now I don't see a punbb 1.3.3 until maybe the end of 2009. I will rather see more mod finished and maybe an upgrade routine that does not truncate all of your 1.2 forum text in your DB(migration to UTF-8 is broken and should not ship).

Of course there is place for all programing languages, but maintaing 2 code base is like shipping a forum with custom implementation of php5 in php4, a lost of time in my view. "Reuse as much as you can" should be the idea.

Good luck and let see how this evolves

Registered: 2006-03-03Posts: 86Re: PunBB.NET beta

I'm sad to see your development team broken in half, this will only slow down PunBB development. I've never heard of a company support a web product for 2 platforms. sad Especially if you try to keep them at the same level with the same features, I just dont see that working out good at all. Building more extensions for PunBB would of increased its popularity a lot more than any .NET version. neutral

From: RussiaRegistered: 2007-12-13Posts: 642Re: PunBB.NET beta

PHP and .NET programming skills are very different. PunBB.NET devs are not too good in PHP and we are not good in .NET. So we couldn't just join .NET-team to PHP-team - this would be useless. And vice versa.

Registered: 2006-06-07Posts: 80Re: PunBB.NET beta

So why don't you hire two more php devels? I have no problem against ASP, but I only have fear that punbb 1.4 will take the same time to develop that 1.3 did.

If .net developers are not hired, then just ignore this message wink

Very nice indeed.

I must say, .NET 1 seemed a bit crappy, but I've had a chance to use .NET 3.5 at work, with VS2008, and it is AWESOME

Also note that it is not ASP, but ASP.NET. Worlds of difference between them

"Take what you need and leave the rest, but never should have taken better"

"Take what you need and leave the rest, but never should have taken better" is a refrain from "The Night they Drove Old Dixie Down", one of the best-known songs of The Band.

Levon Helm, the revered drummer and singer of the band the Band has retained the core band for more than three decades, "peacefully" died on 19 April.

From CNN:

Born in Elaine, Arkansas, in 1940, the son of a cotton farmer, Helm rose to fame in the late 1960s and 1970s as a member of The Band, a folk rock band.

His voice, Soul, drawling highlighted many successful recordings, such as "The Weight", "The Night they Drove Old Dixie Down" and "Up on Cripple Creek".

Helm, 71, was diagnosed with throat cancer in 1998. He fell on hard times as the cancer took his voice and medical expenses threatened his home.

"You got to choose one-pay medical bills or pay the mortgage. Most people cannot do both, and I am no different, "said CNN in 2010.

So The Barn, as the residence is known around Christmas foster helm of Woodstock, New York, became the setting that calls a "last celebration". Not at all. Instead, The Barn became the Centre of an unlikely and unrivaled rock 'n roll revival.

It was there that Helm regularly hosted the midnight ramble, weekly concerts, which attracted sell-out crowds and media all-star. The result not only paid the Bills but also led to a recrudescence creative helm, with his collaborations with Grammy winning album produced back to back: "Dirt farmer" and "the 2007 Electric Dirt" of 2009.

"If I had my way about it, we probably would do it every night," said helm. "I'm not tired of it."

From Wikipedia:

Helm remained with The Band until their 1976 farewell performance, The Last Waltz, which was recorded in a documentary film director Martin Scorsese (an excerpt is embedded above). Many music fans know Helm through his appearance in the concert film, a performance notable for the fact that Helm's vocal tracks appear substantially as they sang during a grueling concert.

In the 1990s, the helm was diagnosed with throat cancer suffering hoarseness. Advised to undergo laryngectomy, helm instead underwent an arduous regimen of radiation therapy at Memorial with Cancer Center in New York City. Although the tumor was successfully removed, the rudder's vocal cords were damaged, and his powerful tenor voice and has been replaced by a quiet RASP. Initially Helm only played drums and relied on guest vocalists at the Rambles, but Helm's singing voice became louder. The January 10, 2004, she sang once again of his ramble sessions. In 2007, during production of dirt farmer, Helm estimated that his singing voice was 80% recovered.

The midnight ramble was an outgrowth of an idea Helm explained to Martin Scorsese in The Last Waltz. Earlier in the 20th century Helm explained, traveling medicine shows and music shows which Assassinators of F.S. Walcott rabbit, featuring African-American blues singers and dancers, would put futile performances in rural areas. This was turned into a song by the band, "The W.S. Walcott Medicine Show," with the name altered so the lyric was easier to sing.

"After the finale, which would have the midnight ramble," Helm told Scorsese. With young children off the premises, picked up the show: "the songs would get a bit more juicy. Jokes would get a little more fun and the best dancer would really get down and shake it a few times. A lot of rock and roll duck walks and moves came from that. "

Helm refused to play "The Night they Drove Old Dixie Down" by 1976, though he continued to perform "Midnight rambles" several times a month in his private residence in Woodstock, New York.

Argo' Trailer: Ben Affleck Rescues Iranian Hostages

Argo Trailer Ben Affleck
Less than 10 years ago, Ben Affleck was the laughing stock of Hollywood, thanks to the commercial and critical bomb known as "Gigli." Thankfully -- for Affleck and for audiences -- it seems like those days are far, far behind him.Click Here!
In his next film, "Argo," Affleck stars as Tony Mendez, a C.I.A. officer who helped extract six American hostages from Iran in 1979, by making them pose as a Canadian film crew. The movie, which Affleck also directed, is based on the real-life story that was documented in a 2007 Wired article by Joshuah Bearman.

In the first trailer for "Argo," Ben -- in his best disco attire -- orchestrates the C.I.A.'s extraction during the Iranian hostage crisis. Fittingly, Aerosmith's "Dream On" plays in the background. You can check out the trailer above.
"Argo," which also stars Bryan Cranston, John Goodman and Alan Aarkin, hits theaters September 14. This will be second the third film directed by Affleck, who received critical acclaim with his last movie, "The Town."
(Correction: We originally referred to "Argo" as the second film directed by Affleck. It is actually his third. Thanks to Twitter user @Ally20791 for pointing that out.)

Those who cannot learn from history are doomed to repeat it.

Box Office: 'Avengers' Makes Billion Dollars Worldwide

AppId is over the quota AppId is over the quota
LOS ANGELES — "The Avengers" is taking a page out of Superman's comic book – flying faster than a speeding bullet to the billion-dollar mark at the box office.
The superhero blockbuster took in $103.2 million to lead for a second-straight weekend, raising its domestic total to $373.2 million, according to studio estimates Sunday.
With $95.4 million more overseas, "The Avengers" lifted its international receipts to $628.9 million and a worldwide haul of just over $1 billion, only 19 days after it began rolling out in some markets.

"You never think that it can happen this quickly," said Dave Hollis, head of distribution for Disney, whose Marvel Studios unit produced the ensemble film after a long buildup in its solo superhero outings. "You hope you can get to this day, and the fact that it is happening this early is a testament to a lot of work that went in on the Marvel side over the last six years to get us to a place where people wanted to see the Avengers assemble."
"The Avengers" easily fended off Johnny Depp and Tim Burton's vampire romp "Dark Shadows," which had a so-so domestic start of $28.8 million to finish a distant No. 2.
That's far below such past Depp-Burton collaborations as "Alice in Wonderland," which opened with $116.1 million, and "Charlie and the Chocolate Factory," which debuted with $56.2 million.
"Dark Shadows" added $36.7 million in 42 overseas markets for a worldwide total of $65.5 million.Click Here!
"The Avengers" was the first movie ever to pull in more than $100 million domestically in its second weekend, passing the previous best of $75.6 million for "Avatar." The film also topped $300 million domestically Saturday after just nine days in release, beating the previous record set by "The Dark Knight," which hit that mark in 10 days.
Already the year's biggest hit worldwide, "The Avengers" is on the verge of passing "The Hunger Games" at $386.9 million to become the top-grossing film domestically for 2012.
Revenue for "The Avengers" was off just 50 percent from the film's domestic debut of $207.4 million the previous weekend, a remarkable hold given how big it started.
A round-up of such Marvel idols as Iron Man (Robert Downey Jr.), the Hulk (Mark Ruffalo), Captain America (Chris Evans) and Thor (Chris Hemsworth), "The Avengers" has shot past the revenues that its solo superhero predecessors took in for their entire runs. The best of those domestically was "Iron Man" with $318.4 million.
"There has been a surprise around every corner with this picture in terms of how high is high and how big is big," Disney's Hollis said.
Inspired by the supernatural soap opera that debuted on TV in the mid-1960s, "Dark Shadows" stars Depp as an 18th century vampire who is freed after two centuries of burial and returns to his ancestral homestead in the 1970s, aiming to rebuild the family fortunes.
The TV show has only a cult following, so the Warner Bros. update relied on the lure of a reunion between frequent collaborators Depp and Burton taking on another otherworldly tale. But "Dark Shadows" left both critics and audiences cold, failing to make much of a dent in the intense appeal of "The Avengers."
"Certainly, more is better, but it was a busy weekend, especially with `Avengers' doing $100 million in its second weekend," said Dan Fellman, head of distribution at Warner Bros. "The box office can only expand so much, and that was a hard one to anticipate. Those numbers are staggering."
Fox Searchlight's crowd-pleaser "The Best Exotic Marigold Hotel" broke into the top-10 in its second weekend as it expanded from a handful of cinemas to 178 theaters. The film took in $2.7 million to finish at No. 8.
"The Best Exotic Marigold Hotel" features Judi Dench, Bill Nighy, Maggie Smith and Tom Wilkinson in a tale of older Brits looking to retire to a cozy life in India.
Also in narrower release of 322 theaters, Eva Mendes' comic drama "Girl in Progress" opened at No. 10 with $1.4 million. The Lionsgate release stars Mendes as a nomadic single mom with a precocious teenage daughter.
"The Avengers" again provided the bulk of Hollywood's business. Overall domestic revenues totaled $172 million, up 23 percent from the same weekend last year, when "Thor" led with $34.7 million, according to box-office tracker Hollywood.com.
Domestic receipts for the year are at $3.83 billion, 17.6 percent ahead of last year's with a huge summer lineup yet to come.
Hollywood.com analyst Paul Dergarabedian said he expects Hollywood to break the summer revenue record of $4.4 billion it set last year and top its all-time annual high of $10.6 billion from 2009.
"I think we will surpass that given the strength of just the first two weeks of the summer and the strength of the films on the way," Dergarabedian said. "Records are just made to be broken this summer and this year."
Estimated ticket sales for Friday through Sunday at U.S. and Canadian theaters, according to Hollywood.com. Where available, latest international numbers are also included. Final domestic figures will be released Monday.
1. "The Avengers," $103.2 million ($95.4 million international).
2. "Dark Shadows," $28.8 million ($36.7 million international).
3. "Think Like a Man," $6.3 million.
4. "The Hunger Games," $4.4 million ($2.4 million international).
5. "The Lucky One," $4.1 million ($2.5 million international).
6. "The Pirates! Band of Misfits," $3.2 million ($2.2 million international).
7. "The Five-Year Engagement," $3.1 million ($1.7 million international).
8. "The Best Exotic Marigold Hotel," $2.7 million ($1.9 million international).
9. "Chimpanzee," $1.6 million.
10. "Girl in Progress," $1.4 million.

Estimated weekend ticket sales at international theaters (excluding the U.S. and Canada) for films distributed overseas by Hollywood studios, according to Rentrak:
1. "The Avengers," $95.4 million.
2. "Dark Shadows," $36.7 million.
3. "American Reunion," $15.6 million.

7. "As One," $2.6 million.

'Battleship' Poster Parody: Taylor Kitsch Faces Off Against Rich Uncle Pennybags

AppId is over the quota AppId is over the quota
If you think a movie based on the game "Battleship" is silly, you're not alone. The folks at Screen Crush have just re-imagined "Battleship"-- a film about an alien invasion -- as a fight against classic board-game characters. (Enter Rich Uncle Pennybags from Monopoly and the King of Candy Land.)
The spoof posters even come with catchy tag lines: Monopoly: "The Battle for Earth Begins at Go" and Jenga: "Attack the Block."
If the current Rotten Tomatoes score for the movie is any indication, a giant Pennybags or angry Operation patient wreaking havoc in the ocean might be more entertaining than the real movie.
Granted, fans shouldn't be laughing too hard: several of these games acutally have movies in the works, including Monopoly from director Ridley Scott and a Candy Land flick starring Adam Sandler.
You can check out a few of the posters below. For the entire collection, head to Screen Crush.
"Battleship" hits theaters nationwide this Friday.

I was pretty excited to try the new addition to the Pomega5 line.
The Pomega5 line infuses antioxidants from pomegranates grown in the Mediterranean, and according to one study, pomegranate seed oil can rejuvenate the skin by boosting cell production. In addition to pure Omega 5, The Daily Revitalizing Concentrate Ampoules contains a blend of oils including jojoba, rosehip, and grapefruit oils, a concentration that the makers claim heals the skin by reducing wrinkles, scars, and dark spots.

I used the Daily Revitalizing Concentrate Ampoules twice a day in lieu of my regular moisturizer for three weeks. I tried the ampoules right before bed, while my face was still damp. The first time, I made the mistake of using an entire capsule on my face, which left my skin shiny. (Luckily, I made that mistake at night, because had it been the morning rush, I would've been waiting impatiently for my skin to dry so I could apply makeup.) My skin did absorb most of the oil eventually, but it was still left with a bit too much shine, so I decided to use half a capsule each time in the future.

Once I adjusted my dosage, my concerns about using oil on my face were abated. My skin absorbed the oil in about a minute. Within three days, my face was much smoother, specifically near the corners of my mouth, which gets extremely dry. Also, I noticed that my skin had a glow, though I needed to keep my oil-absorbing sheets handy for my forehead and nose. Unfortunately, these ampoules didn't reduce a small scar on my cheek, which was what I really wanted. (Note: The ampoules are about the size of a pill, and you have to pull at both ends to apply the product. This can get quite tricky if your hands are wet, so keep a pair of scissors nearby if you plan on using the ampoules straight out the shower.)

The Daily Revitalizing Concentrate Ampoules can also be used on cuticles, so I decided to give it a shot. One capsule was enough for all ten fingers, and I was impressed that this product made my cuticles looked softer right away—without leaving an excess of oil behind like other cuticle treatments. The portability of the ampoules was a plus; it took just two minutes to treat both hands while standing on a moving rush-hour subway car.

These ampoules are a good idea if you're looking for a natural product for dry and uneven skin. But if your skin is on the oiler side, follow my lead and use just half at a time. Since these ampoules are essentially oil, it may be best to keep them away from your T-zone if that area tends to get slick. Since my combination skin gets oily during the summer, I won't use this product when the weather gets warmer, but at least I know now what to use next winter.
